CMS Star Ratings
Every month we pull current CMS star ratings for Arkansas nursing homes so families have a starting point. These are the facilities at the top and bottom of the scale right now.
A 5-star rating does not mean your loved one is safe. Abuse, neglect, and financial exploitation happen in highly rated facilities. Star ratings measure staffing levels, inspection results, and quality metrics. They do not measure whether staff treat residents with dignity. They do not guarantee your loved one will not be harmed.
Use this page as a starting point only. Visit the facility unannounced. Talk to residents and their families. Read the full inspection report on Medicare Care Compare. Trust what you see with your own eyes over any number on any rating system.
If something feels wrong, it probably is. Document it. Report it. You do not need proof to file a complaint.
Do not use this page as your only source of information. Star ratings are a starting point, not a verdict. Always do your own research. Visit Medicare Care Compare directly, read the full inspection reports, and compare at least two or three facilities based on your specific needs or your loved one’s medical and personal needs.
What matters to one family may not matter to another. Location, specialty care, memory units, language spoken by staff, proximity to family, and visiting hours all affect quality of life in ways no star rating measures.

The Only Facility in Arkansas with 5 Stars Across All Four CMS Categories
As of March 2026, only one facility in Arkansas holds a 5-star rating in Overall, Health Inspections, Staffing, and Quality Measures at the same time.
This is a data point, not an endorsement. Even the highest rated facility in the state is not immune to individual incidents of abuse or neglect. If you place a loved one here, stay involved, visit often, and document anything that concerns you.

The red hand icon on Medicare Care Compare identifies a Special Focus Facility (SFF). CMS assigns this designation to nursing homes with a history of serious, repeated quality failures. These facilities face enhanced federal oversight and must show measurable improvement or risk termination from Medicare and Medicaid. An SFF designation means CMS has documented a pattern of harm, not a single incident. If your loved one is in one of these facilities, request the full inspection report today and contact your Long-Term Care Ombudsman.
