Text messages between two licensed practical nurses at Colonial Heights Rehabilitation and Nursing Center in Virginia exposed a system of intentional neglect. 18 employees arrested. A 74-year-old woman dead. Staff falsified medical records to cover what they didn’t do.

The Text Messages

A text exchange between two LPNs at Colonial Heights revealed the culture inside the facility. One nurse wrote to another about the medication pass for nonverbal residents. The message was clear: residents who couldn’t speak up didn’t receive their medications. And staff treated it as routine.

The Death

A 74-year-old woman with cerebral palsy and diabetes was left in her own waste for days. She was not repositioned. She was not changed. She received incorrect medications. She developed sepsis. She died.

Charting Care You Didn’t Give

Prosecutors allege staff engaged in “charting care you didn’t give”, a practice of falsifying electronic health records to show assessments and treatments that never happened. Search warrants revealed staff calling patients names, urging each other to lie in documentation, and evidence of narcotic sales involving residents.

The Arrests

18 employees received warrants. 16 were in custody by December 19, 2024. Four face felony charges of abuse and neglect resulting in death: Shakima Brewer (Night Nurse, denied bail), Danielle Harris (Nurse Trainer), Shawonda Jeter (Administrator), and Kamesha Kittrell (Director of Nursing).

The facility, owned by Innovative Healthcare Management LLC (which operates 5 other Virginia facilities), carried a 1-out-of-5 star CMS rating, placing it in the bottom 15% nationally. Virginia’s Attorney General created an Elder Abuse Investigation Center for Central Virginia in response to this case.
